Our Care Access Nurse is here to guide you through the healthcare system and improve your access to care services. That includes explaining the different services York Hospital offers, helping you find primary care and specialty providers, providing clinical guidance, and answering your questions about follow-up care. If you’re here seasonally or on vacation, we can also explain your care options, and if you’re unable to reach your provider’s office about a particular concern, we can help.
There are many programs, services, and resources offered by the hospital and community that can help improve your quality of life — and our licensed social worker is happy to connect you to them! This can include:
Our Prescription Assistance Navigator is ready to ease your mind about the added costs of prescription medications. Our Prescription Assistance Program offers patients information and help lowering prescription drug costs. We can educate you about pharmaceutical company assistance programs, state and federal programs (including Medicare Part D), and lost-cost generic drug programs.
Our friends at the Care Access Call Center are ready to help. They handle provider referrals and pre-certification, as well as scheduling of testing, procedures, and patient transportation.
